Apple Tree Root Rot Confirmation

I think I know the answer to this, but it would be nice to get a second opinion. I think I have root/crown rot on my poor apple tree. Towards the end of last year, the leaves on the tree were turning a bit yellow but the fruit was perfectly fine, this spring it started to bud and flowered but I noticed that there were very few leaves and they were small. Now most of the tiny leaves fell off and the ones that are left on the tree are slowly turning yellow, I did a little research and then dug at the base of the tree, I attached a picture of what it looks like. I would love to get an opinion of what is going on. Thanks.
