Sick maple

Hello all. I planted a maple tree late last fall. Half the leaves were off when planted. I watered, watered, and watered until freezing temps hit. This spring it woke up great, fully leafed out and started growing new growth on all the tips of branches. About 2 or 3 weeks ago I noticed a large amount of leaves had dark spots and the ends have stopped growing completely and the leaves on the tip of each branch fallen off. We have had pretty consistent rain and I have watered when I felt it was a bit dry. Is it diseased?

Can you get a photo of the base of the tree? It may help to identify the problem.

Top of root ball was even with soil when planted. I cross cut bottom and slit sides of roots also. It was in a 10 gal pot not burlap also if that helps
