Leek problems help!

I recently transplanted some leeks I bought from a garden store into my vegetable garden, every couple days or so a new leek will be fallen over and be very rubbery and the leaves are disconnected from the roots. I am losing many leeks and onions to this and was wondering if anyone could offer some help or insight into this problem?
