Hi folks has anyone seen this before?

I sowed bell pepper seeds from the packet, planted out and got this.

Two different peppers on the same plant.

Had to have been a genetic fault between two hybrids. Let the peppers mature and save the seeds. You may have something there.

It's called hybrid peppers. It's occurs when pollen from one pepper plant is used to pollinate the flower of a different pepper variety. It results in seeds that have genetic material from both parent plants.
