Hey everyone, first time post!

Hoping you can help me identify what is wrong with my plant and how I can help it?! I'm not even sure what type of plant it is. Its been like this for about a month now, looking pretty sad and hurt :(
I've added fertiliser to the pot about 2 weeks ago but still no results. It doesn't get direct sunlight on it but sits in a well lit room

Any recommendations would be greatly appreciated!

Chinese evergreen (Aglaonema) and it appears to be transplanted in a container which holds too much water. That being said, you can cut the water and extend the time between waterings and that may help. Allow for the top 2 inches to dry before watering. There is no need to fertilize a plant in new potting soil unless the mix is 6 months old or older, or it is soilless mix, because new potting soil will have a charge of nutrients already in the mix. . Chinese evergreen uses a very tiny amount of nutrients almost none at all.
