Need to ID....before diagnosis.

Hello all, I'd like to try identifying this plant. It was already planted when we moved to the house so we have no idea what it is.

For the past two springs I've found that several of the stems of the plant die or partially die, which leaves it looking pretty ratty. Some stems are dead from the base to the tip, some just towards the tip and some have dead portions in the middle and then have green growth at the end.

These pictures are after I've cut most the dead out.
