Does anyone know what these growths are? Don’t look like new leaves so I assume roots and maybe it needs a bigger container?

Thanks for any help.

These are aerial roots. If I transplant my phalaenopsis, I choose a transparent pot. This allows you to see the state of the roots, which are located in the lower part of the plant. The aerial roots on your phalaenopsis look healthy.
I would transplant into a transparent pot a little larger. The substrate is a slightly moist pine bark. Watering - once a week immersion in warm water for 15-25 minutes. Or spray with warm water.
