Help figuring out please

I have this cool looking plant that was given to me about 3 or 4 years ago and it just recently became very unhealthy looking. The person I received it from is no longer around to ask but I believe it is 2 plants that were grafted together that up until now had no problems with and I haven't done anything different to cause it to get sickly. Any info on what it is and or safe it would be appreciated

From what I can see your plant is a succulent named Mermaids Tail. I think it has received way too much rainfall for the past month or two. It is probably a goner if it doesn't stop raining.

It has been raining a lot lately and no sign of it stopping anytime soon I really don't want it to die since it was a gift from a friend who has passed away. Should I put it in a pot and bring it inside

Yes. Remove all of the old soil and replace with a succulent mix.

Thank you for your help I've got it all done I'll update on how it goes, hopefully it will make it through this
